What is the Patient Portal (PatientAccess)?
A secure online tool to access your important documents (e.g. statements, treatment plans), view and confirm appointments, verify personal and medical information, update records and forms, and pay clinic fees.
How do I register?
- Register online yourself: use the patient's birth date, legal first name, and email or mobile number on file.
What do I need to sign up?
- You must be an existing patient or guarantor at the Faculty of Dentistry.
- An email address OR mobile number on file. Confirm this with Front Desk if you are unsure.
- Parents registering for a child must use the parent email OR mobile number on file.
- Need multiple parents/guardians to have access? Tell our Patient Portal specialist.
- Note: Upon first login, you will be assigned an auto-generated username. Please remember this for now, but you will be able to change your username as soon as registration is complete.
How do I sign in?
Enter your username and password, then send a verification code to your email or mobile number. On first sign-in, accept the Terms of Use.
Two-Factor Authentication (also called 2FA)
You’ll receive a one-time code by email or text message each time you sign in. Be ready to enter this after your password.
Didn’t get the code? Check spam/junk, wait up to 1 minute, then try again.
Forgot username? Use: Recover Username
Forgot password? Use: Reset Password
You’ll be asked for your birth date, legal first name, and email or mobile number on file.
Locked out? After several unsuccessful attempts, access may be temporarily blocked. Don’t worry - call 604-822-2112 and we’ll help.
What can I do?
- Download important documents (e.g., treatment plans, statements, payment receipts, and other shared documents).
- Review personal and medical information and mark items as Incorrect for our staff to update at your next visit.
- View and confirm upcoming appointments.
- View past treatments and appointment history.
- Complete assigned forms or instructions before your visit.
Note: Any x-rays, images, and other medical records will first require a Release of Records consent to be completed before this can be made available on the Patient Portal. Please contact our staff to complete this form.
Confirm an appointment
Go to Appointments → select the appointment → tap Confirm. This will notify your provider that you will be attending the scheduled appointment. For cancellations, please contact your provider directly to let them know that you will be unavailable.
Change your password
Go to Personal Information → Change Username and Password → enter current and new password → Update.

You will need:
- Chart Number (found on the top right section of your statement)
- Legal Last Name
- Legal First Name
- Birth Year
- Email address on file (Patient or Guarantor)

Getting an error during registration?
Make sure you enter the patient's birth date, legal first name, and email or mobile number on file. If you are still receiving an error, please contact us for help. It is possible that the patient's information on file is missing or not up to date.
